In alignment with its focus on nursing quality and continuous improvement, the emergency department at the UCI Medical Center – a Level I adult trauma center, which is also a, Level II pediatric trauma, regional burn center and a Comprehensive Stroke Center – implemented a nurse-led initiative known as Team Discharge. This collaborative model between nurses and
physicians was developed to enhance the timeliness and effectiveness of patient discharges, a critical component to achieving optimal patient flow and bed availability. Team Discharge exemplifies nursing leadership in action—fostering real-time communication, ensuring patients receive and understand discharge instructions thoroughly, and proactively addressing any barriers to a safe transition of care.
