The 12-bed emergency department at Riverside Doctors’ Hospital-Williamsburg has significantly improved door-to-needle times for thrombolytic administration in ischemic stroke patients, now consistently achieving under 60 minutes and often under 45. Historically, low administration volume hindered staff confidence and process refinement. In late 2024, nurse-led improvements transformed the department’s stroke response by transitioning to a new telestroke provider, which enhanced real-time communication, and implementing ED-driven workflow changes. Stroke
boxes are now brought directly to CT, and the nurses, providers and teleneurologists collaborate at the stretcherside for NIHSS assessments. These efforts have streamlined care, reduced delays and strengthened team
coordination.
