To improve response times for critical patients in this increasingly busy 26-bed department, the ED team at Northwell Greenwich Village Hospital implemented a clinical upgrade assignment process. Previously, upgrades were called by triage nurses without assigned providers, leading to inconsistencies and potential delays. The new system created physician-APP teams and assigned upgrades in a round-robin fashion, tracked by the charge nurse and announced overhead. A five-minute response time is expected, with escalation to the physician in charge if needed. Standardized upgrade criteria and regular audits ensure consistent application. The team continues to monitor and refine this process for optimal patient care, and early results show improved response times, enhanced communication and better team cohesion.
