practices that improve care for older adults. Through nurse-led interdisciplinary collaboration, staff education, standardized protocols and targeted quality improvement initiatives, the team reduced restraint use, decreased indwelling urinary catheter utilization and lowered catheter-associated urinary tract infections among patients age 65 and older. These sustained improvements enhanced patient safety, preserved dignity, reduced preventable harm and improved the care experience for the 43-bed ED’s growing geriatric population.

The team implemented evidence-based strategies to reduce CAUTIs through nurse-driven practice changes; standardized catheter insertion and maintenance practices; daily assessment of catheter necessity utilizing a “Why The Foley” form; and interdisciplinary collaboration. Ongoing education, quarterly data review and front-line engagement strengthened accountability and sustained improvement. From the first quarter of 2024 through the fourth quarter of 2025, catheter utilization among patients older than 65 declined from 3.6 percent to 2.5 percent, with five of eight quarters meeting or remaining near the departmental goal of less than 3 percent. These efforts enhanced patient safety, reduced preventable harm and reinforced the team’s commitment to evidence-based practice and continuous quality improvement.
