The 29-bed pediatric emergency department at MUSC Shawn JenkinsChildren’s Hospital created an ultrasound-guided peripheral IV program, beginning at a time when the pediatric ED had no nurses trained in US-PIV placement and little opportunity for training. In 2020, one of the pediatric emergency medicine physicians began offering training on US-PIV placement consisting of one-on-one training and 10 proctored US-PIV placements prior to sign-off for independent practice. Two years after implementation, 10 PED nurses completed training, placing more than 200 US-PIVs. In 2022, the hospital’s pediatric vascular access team experienced a staffing shortage. To address this issue, the PED ultrasound physician lead and ultrasound-trained
PED nurses partnered to expand the program to the entire pediatric hospital. With the use of philanthropic funding, nurses who complete initial training are allotted 16 hours of paid proctored IV placement time with select ultrasound-trained PED nurses. The program has seen great success, with 18 PED nurses and 19 inpatient nurses achieving competency, and 11 PED nurses achieving proctor level competency. Since April 2023, a total of 1,726 US-PIVs have been placed by trained nurses, highlighting the program’s continued growth and success.
