identifying and treating sepsis patients through the integration of informatics-based tools in the electronic medical record as well as a heightened level of education and awareness in triage. Further, primary nurses are given the ability to initiate the sepsis pathway. These changes, along with focused staff rounding, led to a noticeable improvement in every captured measure,
including time-to-sepsis-recognition, initiation of resuscitation, repeat evaluation of patient condition and escalation of level of care when warranted.

The second project is the early identification and treatment of community acquired pressure injuries. This involved in-depth education on staging and treatment of pressure injuries for all emergency department nurses. Additionally, the team developed a dedicated nursing assignment in the department that focuses on skin assessment for all elderly or bedbound patients, as well as those at high risk for skin breakdown, resulting in a marked increase in identification of these wounds and allowing for earlier initiation of treatment.
