The emergency department team at Bethesda North Hospital, a Joint Commission-validated primary stroke and thrombectomy capable center, focused on improving door-to-needle median Tenecteplase times. In early 2024, the organization developed an internal stroke team to meet the guideline for initiating intravenous TNKase within 60 minutes of the patient’s arrival to the ED. The team reduced the door-to-needle median time from an average of 53 minutes to 32 minutes.
The team also developed an evidence-based practice project to create guidelines, provide education and standardize the process of extracorporeal membrane oxygenation in the ED setting. A literature search was performed, and an interdisciplinary team developed educational offerings and created guidelines to perform ECMO in the ED. The interdisciplinary team designed a strategic approach for initiating ECMO in the ED setting that provides the ED staff nurses with an understanding of their role in the care of the patient undergoing ECMO cannulation, determining what patients are candidates for ECMO cannulation, as well as where and how to document appropriately in the electronic medical record platform used by in the emergency department.
